应用血管内皮生长因子拮抗剂预防鼻息肉形成及复发的可能机制

摘    要:背景有大量的文献表明血管内皮生长因子在肿瘤的形成中起重要作用,但少有文献探讨血管内皮生长因子在鼻息肉形成中的作用.目的探讨血管内皮生长因子及其受体在鼻腔生理及鼻息肉形成中的作用.设计以诊断为依据设立对照的实验研究.地点和对象实验在解放军广州军区广州总医院耳鼻咽喉科完成,对象为鼻息肉患者30例,均来自本院耳鼻喉科住院手术患者.6名志愿者为本科因非鼻息肉住院的患者.主要观察指标鼻息肉组织和正常鼻黏膜组织血管内皮生长因子及其受体的表达.结果血管内皮生长因子及其受体在正常鼻黏膜中呈阳性表达,在鼻息肉组织中呈强阳性表达.结论本实验结果提示血管内皮生长因子及其受体参与正常鼻腔生理功能的调节,并参与鼻息肉的形成机制.因而推测鼻息肉术后在鼻腔应用血管内皮生长因子的拮抗剂对鼻息肉的复发有预防作用.

Abstract:BACKGROUND: A large amount of references suggest that vascular endothelial growth factor(VEGF) plays an important role in tumor formation,but there are now few references about the functions of VEGF in the formation of rhinopolypus.OBJECTIVE: To investigate the functions of VEGF and its receptors in nasal physiology and in formation of rhinopolypus.DESIGN: A control experimental study depended on diagnosis was conducted.SETTING and PARTICIPANTS: The experiment was completed in the Department of Otorhinolaryngology, Guangzhou General Hospital of Guangzhou Military Region. The subjects were 30 patients with rhinopolypus who were in-patients operated in the department of othorhinolaryngology. 6volunteers were also in-patients with non-rhinopolypus.MAIN OUTCOME MEASURES: The expression of VEGF and its receptors in polypous tissue and in normal nasal mucosa tissues.RESULTS: The expression of VEGF and its receptors was positive in normal nasal mucosa tissues and strong positive in polypous tissue.CONCLUSION: The experiment suggests that VEGF and its receptors can participate in the regulation of normal nasal physiological functions and in the mechanism of formation of rhinopolypus. So it is inferred that the recurrence of rhinopolypus can be prevented by using VEGF antagonists in nasal cavities after nasal polypectomy.
