急性白血病患者脑脊液β_2微球蛋白的临床意义

目的　探讨急性白血病 (AL)患者脑脊液 β2 微球蛋白 (CSFβ2 -MG)的临床意义。方法　采用放射免疫法对 62例AL患者进行CSFβ2 -MG、细胞学检测 ,并对伴有中枢神经系统白血病 (CNSL)的 2 5例患者的CSFβ2 -MG动态观察。结果　 2 5例合并CNSL患者CSFβ2 -MG明显高于正常对照组和不合并CNSL组 (P均 <0 .0 1) ,而不合并CNSL组与正常对照组比较差异无显著性 (P >0 .0 5 )。 2 5例合并CNSL患者经治疗后 13例病情缓解 ,CSFβ2 -MG下降 ,与治疗前相比差异有显著性 (P <0 .0 5 ) ;12例未缓解的患者 ,CSFβ2 -MG与治疗前比较差异无显著性 (P >0 .0 5 )。结论　检测CSFβ2 -MG对CNSL的早期诊断有重要意义 ,动态检测CSFβ2 -MG可作为CNSL治疗疗效判定的指标。

The clinical significance of CSF β2-MG in acute leukemia

Objective To study the clinical significance of the cerebrospinal fluid β 2-microglobulin (CSF β 2-MG) in acute leukemia (AL). Methods A radioimmunoassay was used to detect the CSF β 2-MG in 62 patients with AL and a cytological examination was also done. A dynamic observation on CSF β 2-MG in 25 patients complicated with CNSL had been undertaken as well. Results The CSF β 2-MG in 25 patients complicated with CNSL was statistically higher than that in the control group and that in the uncomplicated with CNSL group (all the P values were <0.01), and there was no statistically significant difference between the uncomplicated with CNSL group and the control group (P>0.05). 13 of the 25 patients complicated with CNSL had got remission after treatment, so the CSF β 2- MG decreased, and there was statistical difference in comparison to the levels treated before (P<0.05); 12 of them which had not got remission, there was no statistical difference in comparison of the CSF β 2-MG to the levels treated before (P>0.05). Conclusion CSF β 2-MG is a usful marker in early diagnosis of CNSL, and the dynamic detection of CSF β 2-MG can be an index to judge the efficacy of the treatment.
