摘    要:目的探讨血浆中B型钠利尿肽(BNP)水平与心力衰竭的关系。方法选取左心室收缩功能衰竭患者100例(试验组),对照组100例,心功能正常。试验组依据美国NYHA标准分级:Ⅰ级患者16例,Ⅱ级患者24例,Ⅲ级患者48例,Ⅳ级患者12例。采用放射免疫法测定血浆BNP水平,心脏彩色多普勒超声诊断仪测量左室射血分数(LVEF);Swan-Ganz球囊漂浮导管检测肺动脉压(PAP)。结果试验组BNP水平明显高于对照组,差异统计学意义(P<0.05)。血浆BNP与LVEF呈负相关(P<0.05),与肺动脉压力(PAP)正相关(P<0.05)。结论左心室收缩功能衰竭时患者血浆BNP升高,与衰竭程度正相关;血浆BNP浓度与肺动脉压力(PAP)呈正相关、LVEF呈负相关,存在线性关系。

Abstract:Objective To explore the correlation between plasma levels of brain natriuretic peptide (BNP) and chronic heart failure (CHF). Methods One hundred patients with left ventricular systolic function failure due to coronary atherosclerosis including 70 males and 30 females (age range: 32 -78yr) were enrolled as trial group,and the other 100 healthy subjects including 80 males and 20 females (age range: 34 -58yr) were enrolled as control group. The patients in trial group included 16 cases at grade I ,24 cases at grade ]] ,48 cases at grade m and 12 cases at grade IV according to the NYHA classification standard. The plasma levels of BNP were detected by radio immune assay,left ventricular ejection fraction was determined by Doppler echocardiography, and pulmonary artery pressure (PAP) was detected by Swan-Ganz catheter. Results The plasma levels of BNP in trial group were significantly higher than those in control group ( P 〈0.05 ). The plasma BNP was negatively correlated to LVEF ( P 〈0.05 ), however, which was positively related with PAP ( P 〈0.05 ). Conclusion The plasma levels of BNP in patients with left ventricular systolic function failure are increased, which are positively correlated to failure degree;plasma levels of BNP were positivekly related with PAP, however, which are negatively correlated to LVEF with a linear relationship.
