奥曲肽联合奥美拉唑治疗门脉高压并上消化道出血术后再出血45例疗效观察

目的 观察奥曲肽联合奥美拉唑治疗门脉高压并上消化道出血术后再出血的疗效.方法 将90例门脉高压症术后患者随机分为治疗组45例和对照组45例.治疗组给予奥曲肽联合奥美拉唑治疗,对照组单用奥美拉唑治疗.观察二组的止血效果.结果 治疗组总有效率为93.3％,对照组为77.8％(P＜0.01);止血时间治疗组为(21.76±5.35)h,对照组为(27.46±5.73)h(P＜0.05);72h后再出血率治疗组为6.7％,对照组为22.2％,治疗组输血量为(1.67±0.54)U,对照组为(2.88±0.52)U,差异均有统计学意义(P＜0.05).结论 奥曲肽联合奥美拉唑治疗门脉高压并上消化道出血术后再出血疗效显著,较单用奥美拉唑有止血速度快、输血量减少、早期再出血发生率低的特点.

THE CLINICAL EFFECT OF OCTREOTIDE COMBINED WITH OMEPRAZOLE IN THE PORTAL HYPERTENSION PATIENTS WITH POSTOPERATIVE REHAEMORRHAGIA IN UPPER GASTROINTESTINAL BLEEDING

Objective To observe the efficacy of octreotide combined with omeprazole in the portal hypertension patients with postoperative rehaemorrhagia in upper gastrointestinal bleeding.Methods 90 cases of the rehaemorrhagia of upper gastrointestinal bleeding were randomly divided into treatment group(45 cases)and control group(45 cases).Treatment group received octreotide plus omeprazole,while control group was treated with omeprazole alone,observed the hemostatic effect between these two groups.Results The total effective rate of treatment group was significantly higher than that of control group(93.3%VS 77.8%,P<0.01).The hemostasis time in treatment group was shorter than that in control group (21.76±5.35)h vs(27.46±5.73)h,P<0.05)];The blood transfusion requirement and the rebleeding rate after the first 72 hours in treatment group were less than that in control group(1.67±0.54)U VS(2.88±0.52)U;6.7%vs 22.2%)],the differences were statistically significant(P<0.05).Conclusion The combination of octreotide and omeprazole infusion is more effective than omeprazole alone in the portal hypertension patients with postoperative rehaemorrhagia in upper gastrointestinal bleeding.it can lead to speeder up hemostasis,fewer blood transfusion requirements and prevent early rebleeding effectively.
