The Long term Outcome of Microsurgery for Idiopathic Trigeminal Neuralgia

Objective: To demonstrate the cause and optimal treatment for idiopathic trigeminal neuralgia(ITN). Methods: 95 patients were treated with micro vascular decompression(MVD) for ITN caused by neurovascular compression; 37 underwent rhizotomy of partial afferent root of trigeminal nerve for ITN without neurovascular compression. Clinical presentations, surgical findings, and clinical outcomes were analyzed. Results: After following up for 6 months to 15 years (mean 5.4 years), of the 132 patients who underwent microvascular decompression or rhizotomy of partial afferent root of trigeminal neuralgia, 130(98.5%) had excellent improvement including free of pain without medication(97.0%) and occasional pain that did not require long term medication(1.5%). Two patients(1.5%)had recurrences during study period, their pain, however, was improved after successful subsequent MVD procedures. Conclusion: MVD treatment for ITN caused neurovascular compression and rhizotomy of partial afferent root of trigeminal nerve for ITN without vascular compression are safe and effective with a high rate of long-term improvement.
