鼻腔术后填塞对动脉血气影响的研究

目的 探讨鼻内手术鼻腔填塞前后对动脉血气分析影响的发生机制及其临床意义.方法 选择20例做鼻内手术的患者,用血气分析仪分别对填塞前、后24h、抽出填塞物后24h的动脉血进行分析.结果 ①抽出填塞物后24h的PaO2明显高于术前、术后24h(p<0.05);②鼻腔填塞前、后及抽出填塞物后24h的PaCO2、pH、HCO3-差别有非常显著性(p<0.01);③抽出填塞物后24h的O2Hb、P50明显高于术前、术后24h(p<0.05).结论 鼻腔、鼻窦阻塞性炎症的患者应尽早治疗,对术后鼻腔填塞者要密切观察因缺氧诱发临床症状,尤其是中老年伴有心血管疾病的患者.

The Effect of Nasal Packing on Arterial Blood Gases after Surgical Operations

Objective To study the generation mechanism and its significanceof the effect on arterial blood gases before or after nasal packing.Methods The arterial blood of twenty cases was observed by the ABLTM520 Blood Gas System,which was made in Denmark.The samples were collected in three situations:24 hours before and after nasal packing, 24 hours after picking out the packing. Results ①As regards to PaO2,there was a significant increase after picking out the nasal packings(p<0.05);②There were very significant difference in the mean values for PaCO2、pH、HCO3- in cases 24 hours before, after packing the nose and picked out the packing(p<0.01);③There were significant increase in the mean values for O2Hb,P50in cases after picking out the nasal packing (p<0.05).Conclusions The cases with obstructive inflammation in nasal cavity or sinus ,which must have a treatment as early as possible.And the cases should be closely observed after the surgical operations with a packing nose , especially in old age and with cardiac disease.
