注射用血凝酶在改良乳腺癌根治术中的应用

目的 观察注射用血凝酶在改良乳腺癌根治术中及术后的止血效果.方法 将80例行改良乳腺癌根治术的乳腺癌患者完全随机分成血凝酶组和对照组,各40例.血凝酶组于术前1小时肌内注射血凝酶1U,术前15 min静注1U,术后2d每天肌内注射1U；对照组采用0.9％氯化钠注射液浸泡纱布.记录、观察并对比2组创面止血时间、单位面积渗血量、术中出血量(记录吸引器液体量及纱布差值)、术后2d的引流量及凝血功能.结果 血凝酶组切口止血时间、切口单位面积渗血量、术中出血量、术后2d的引流量均明显少于对照组[止血时间:(102.3 ±24.2)s比(118.8±30.1)s;切口单位面积渗血量:(0.3 ±0.1)g/cm2比(0.4±0.2) g/cm2；术中出血量:(14.2±3.6)g比(20.9±5.1)g；术后2d引流量:(4.5±0.4)ml比(8.2±0.6)ml],差异均有统计学意义(均P＜0.05).而2组术前和术后24h凝血功能差异无统计学意义(P＞0.05).结论 注射用血凝酶在改良乳腺癌根治术中的止血效果明显,能减少术中、术后出血,缩短手术时间,且对凝血功能影响不大.

The research of Hemocoagulase Atrox's hemostasis function in modified radical mastectomy

Objective To observe the Hemocoagulase Atrox's hemostasis effect during and after modified radical mastectomy.Methods Eighty breast cancer patients undergoing the modified radical mastectomy were divided into two groups and each group included 40 cases.Hemostasis time,wound's bleeding volume per centimeter,bleeding volume during the operation,drainage 2days after operation and it's effect on Blood coagulation function (PT,APTT,PLT,Fib) were observed.Results The hemostasis time,wound's bleeding volume per centimeter,bleeding volume during the operation and drainage 2days after operation in the Hemocoagulase Atrox group were shorter than those of the control group significantly.And at the same time,the blood coagulation function 24 hours before and after operation of two groups were not different significantly ( P ＞ 0.05 ).Conclusions The hemostasis function of Hemocoagulase Atrox for Injection in modified radical mastectomy is significant.It can reduce the bleeding volume during and after the operation,shorten the operation time.But it's effect on blood coagulation function is linited.
