血液检验红细胞参数在贫血中的鉴别诊断价值

摘    要:目的探究血液检验红细胞参数在贫血中的鉴别诊断价值。方法选取54例贫血患者作为观察组,另选取同期健康体检者54例作为参照组。两组均以美国贝克曼库尔特全自动生化分析仪进行血液检验。分析观察组患者诊断结果 ,并比较观察组和参照组、巨幼细胞性贫血与缺铁性贫血患者的各项红细胞参数血红蛋白(Hb)、平均红细胞体积(MCV)、红细胞分布宽度(RDW)、红细胞计数(RBC)、平均红细胞血红蛋白含量(MCH)]。结果经临床诊断,观察组54例患者中巨幼细胞性贫血患者30例,缺铁性贫血患者24例。血液检验红细胞参数诊断结果显示,巨幼细胞性贫血患者27例,缺铁性贫血患者27例,以临床诊断为金标准,误诊3例,诊断准确率为94.44%(51/54)。观察组RDW(19.56±2.11)%高于参照组的(10.26±1.32)%, Hb(73.26±3.62)g/L、MCV(65.19±3.42)fl、RBC(2.53±0.41)×1012/L、MCH(22.34±1.65)pg/L均低于参照组的(134.26±3.26)g/L、(83.26±3.22)fl、(4.53±0.55)×1012/L、(31.26±1.52)pg/L,差异具有统计学意义(P<0.05)。巨幼细胞性贫血患者的RBC和Hb水平均低于缺铁性贫血患者,而RDW、MCV、MCH水平均高于缺铁性贫血患者,差异具有统计学意义(P<0.05)。结论经血液检验红细胞参数可尽早确诊患者病情,准确掌握贫血患者的血液浓度变化,从而为患者病情诊断与治疗提供可靠依据,其应用价值显著。

Abstract:Objective To investigate the value of red blood cell parameters in the differential diagnosis of anemia.Methods There were 54 anemia patients selected as the observation group,and another 54 healthy subjects as the control group.Both groups were tested with American Beckman Coulter automatic biochemical analyzer.Comparison was made on diagnosis results of the observation group were analyzed,and the red blood cell parametershemoglobin(Hb),mean corpuscular volume(MCV),red cell distribution width(RDW),red blood cell count(RBC),mean corpuscular hemoglobin(MCH)]between the observation group and the reference group,between megaloblastic anemia and iron-deficiency anemia.Results After clinical diagnosis,there were 30 cases of megaloblastic anemia and 24 cases of iron deficiency anemia in 54 cases of the observation group.The diagnostic results of red blood cell parameters by blood test showed that there were 27 patients with megaloblastic anemia and 27 patients with iron-deficiency anemia.With clinical diagnosis as the gold standard,3 cases were misdiagnosed,and the diagnostic accuracy rate was 94.44%(51/54).The RDW(19.56±2.11)%of the observation group was higher than(10.26±1.32)%of the control group,and Hb(73.26±3.62)g/L,MCV(65.19±3.42)fl,RBC(2.53±0.41)×1012/L and MCH(22.34±1.65)pg/L were all lower than(134.26±3.26)g/L,(83.26±3.22)fl,(4.53±0.55)×1012/L and(31.26±1.52)pg/L of the control group,and the difference was statistically significant(P<0.05).The RBC and Hb levels of megaloblastic anemia patients were lower than those of iron-deficiency anemia patients,while the RDW,MCV,and MCH levels were higher than those of iron-deficiency anemia patients.The difference was statistically significant(P<0.05).Conclusion Red blood cell parameters can be used to diagnose the condition of patients as soon as possible,and the changes of blood concentration of patients with anemia can be accurately controlled,thus providing reliable basis for the diagnosis and treatment of patients’condition,and its application value is significant.
