Comparative analysis of total laparoscopic and open distal gastrectomy for advanced gastric cancer

Objective To analyze the efficacy of total laparoscopic and open distal gastrectomy for advanced gastric cancer.Methods A total of 76 patients with advanced gastric cancer were randomly divided into control group and observation group,with 38 cases in each group.The control group was treated with open distal gastrectomy,and the observation group was treated with total laparoscopic surgery.The clinical indicators,radical cure effect,and occurrence of postoperative complications were compared between the two groups.Results The operation time(182.49±5.62)min of the observation group was longer than(160.52±12.40)min of the control group,intraoperative blood loss(61.82±2.19)ml was less than(101.83±4.21)ml of the control group,and postoperative exhaust time(3.15±0.42)d and incision length(5.97±1.24)cm were shorter than(5.42±0.64)d and(18.64±2.84)cm of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in number of dissected lymph nodes,the length of proximal margin and distal margin between the two groups(P>0.05).The incidence of postoperative complications 5.26%of the observation group was lower than 21.05%of the control group,and the difference was statistically significant(χ²=4.145,P=0.042<0.05).Conclusion For patients with advanced gastric cancer,total laparoscopic surgery can not only effectively improve the treatment effect,but also reduce incision infection and improve clinical observation indicators,which has clinical application value.
