增水行舟推拿法治疗小儿热秘实证30例临床观察

目的观察增水行舟推拿法治疗小儿热秘实证的临床疗效。方法选取2019年7月至2020年2月钦州市中医医院小儿推拿科门诊就诊的60例热秘实证患儿,按照随机数字表法分为对照组和治疗组,各30例。对照组口服乳果糖口服溶液,治疗组予增水行舟推拿法,2组均连续治疗7 d后统计疗效。结果治疗组总有效率为96.4%(29/30),对照组为93.3%(28/30),2组比较,差异有统计学意义(P<0.05)。治疗前,2组主、次症积分和总积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后,除对照组主症间隔时间,次症腹胀腹痛、小便短黄外,2组其他主、次症积分和总积分均较同组治疗前明显下降(P<0.05或P<0.01),且治疗组下降更显著(P<0.05或P<0.01)。治疗组中医证候疗效为96.7%(29/30),对照组为90.0%(27/30),2组比较,差异有统计学意义(P<0.05)。结论增水行舟推拿法治疗小儿热秘实证,不仅能明显改善患儿症状和体征,疗效显著,而且患儿及其家长的依从性较好,值得临床推广应用。

Clinical observation on 30 cases of children with heat constipation excess syndrome treated by manipulative techniques of tuina with increasing body fluid to relieve constipation

Objective To observe the clinical efficacy of manipulative techniques of tuina with increasing body fluid to relieve constipation(IBFRC) in the treatment of children with heat constipation excess syndrome.Methods From July 2019 to February 2020,60 cases of children with heat constipation excess syndrome consulted in outpatient service of pediatric tuina department in Qinzhou Municipal Hospital of TCM were chosen and divided into control group and treatment group according to random number table method,with 30 cases in each.The control group was given oral administration of lactulose oral solution,while the treatment group was given manipulative techniques of tuina with IBFRC.We kept statistics about the curative effects after both groups were treated consecutively for 7 days.Results The total effective rate of the treatment group was 96.4%(29/30),that of the control group was 93.3%(28/30),and the difference was statistically significant between the 2 groups(P<0.05).Before treatment, the difference was not statistically significant in the main and minor symptom scores and total score between the 2 groups(P>0.05),and they were comparable.After treatment, except for the interval time between main symptoms, abdominal distension pain and scanty and brown urine in the control group,the scores of other main and minor symptoms and the total score of the 2 groups were significantly decreased compared with those before treatment in the same group(P<0.05 or P<0.01),and the decrease in the treatment group was more significant(P<0.05 or P<0.01).The curative effect of TCM symptom in the treatment group was 96.7%(29/30),and that in the control group was 90.0%(27/30),and the difference was statistically significant between the 2 groups(P<0.05).Conclusion It can not only markedly improve the symptoms and signs of children,but also has good compliance with children and their parents to use manipulative techniques of tuina with IBFRC in the treatment of children with heat constipation excess syndrome,which is worthy of clinical promotion and application.
