SBAR沟通模式在新生儿重症监护病房护理床头交接班中的应用效果

目的探讨个体化现状-背景-评估-建议(SBAR)沟通模式在新生儿ICU护理床头交接班中的应用效果。方法采用便利抽样法,选择2018年2—8月浙江省永康市第一人民医院新生儿ICU的96例患儿作为对照组;选择2018年9月—2019年3月我院新生儿ICU的107例患儿作为观察组。对照组采用常规交接班模式,观察组采用SBAR沟通模式。比较两组护理不良事件发生率、护理人员对患儿病情掌握情况及交接班所用时间。结果观察组患儿护理不良事件总发生率为8.41%(9/107),低于对照组的22.92%(22/96),差异具有统计学意义(χ^(2)=8.2286,P<0.01);护理人员对观察组患儿诊断和病情、治疗、异常化验结果、护理重点、存在风险等病情掌握程度优于对照组,差异有统计学意义(P<0.001);护理人员在护理对照组患儿时进行交接班所用时间为(10.44±1.51)min,长于观察组的(5.32±0.49)min,差异有统计学意义(t=33.1884,P<0.001)。结论SBAR沟通模式应用于新生儿ICU护理床头交接班中能有效减少护理不良事件发生率,提高护理人员对患儿病情的掌握程度,不但有效的保障了患儿安全,而且可缩短交接班时间,进而提高了护理质量,值得推广应用。

Effect of SBAR communication mode in nursing bedside handover of Neonatal Intensive Care Unit

Objective To explore the effect of individualized situation-background-assessment-recommendation(SBAR)communication mode in nursing bedside handover of Neonatal Intensive Care Unit(NICU).Methods Convenience sampling was used to select 96 neonate in NICU of the First People's Hospital of Yongkang City,Zhejiang Province from February to August 2018 in control group,and select 107 neonate in NICU in our hospital from September 2018 to March 2019 in observation group.Control group carried out regular handover mode,and observation group implemented SBAR communication mode.The incidence of adverse nursing events,nurses'knowledge of the neonate's condition and the time spent on handover were compared between two groups.Results The total incidence of adverse nursing events in observation group was 8.41%(9/107),which was lower than 22.92%(22/96)in control group,and the difference was statistically significant(χ²=8.2286,P<0.01).The nurses'mastery of the neonate's diagnosis and condition,treatment,abnormal laboratory results,nursing focus,and risks of in observation group was better than that in control group with a statistically significant difference(P<0.001).Nurses took(10.44±1.51)minutes to transfer in nursing for children in control group,which was longer than(5.32±0.49)minutes in observation group,and the difference was statistically significant(t=33.1884,P<0.001).Conclusions The application of SBAR communication mode in nursing bedside handover of NICU can effectively reduce the incidence of nursing adverse events and improve the nurses'grasp of the neonate's condition.It not only effectively guarantees the safety of the neonate,but also shortens the shift time,so as to improve the quality of nursing care,which is worthy of promotion and application.
