介入再通配合活血化瘀中药治疗输卵管阻塞性不孕症的前瞻性研究

目的研究介入再通配合活血化瘀中药对输卵管阻塞性不孕症的治疗效果。方法前瞻性研究经子宫输卵管造影(HSG)检查证实为输卵管近段完全阻塞511例,随机分为试验组和对照组,两组均经输卵管介入再通术疏通阻塞输卵管,试验组术中术后加活血化瘀中药治疗,对照组为经典介入再通术,不予中药治疗。失访病例72例,实际纳入研究病例439例。经12个月和24个月随访,评价输卵管通畅度、宫内妊娠率和输卵管妊娠率。结果介入后12个月和24个月,试验组的输卵管完全通畅率明显高于对照组,再闭塞率及通而不畅率显著低于对照组;输卵管妊娠率显著低于对照组。结论介入再通配合活血化瘀中药治疗输卵管阻塞性不孕具有良好的临床疗效。

Prospective study on the treatment for the infertile by fallopian tube obstruction with fallopian tube recanalization and blood-activating-dispersing traditional Chinese medicine

Objective To investigate the effects of a therapy involving selective salpingography and fallopian tube recanalization(FTR)and the application of blood-activating-dispersing traditional chinese medicine(TCM)upon infertility caused by fallopian tube obstruction.Methods A total of 511 infertile cases with fallopian tube obstruction were prospectively and randomly divided into experimental group and control group.The experimental group received blood-activating-dispersing TCM during and after FTR,while the control group underwent FTR only.Seventy-two patients were lost during the follow-up,and 439 patients were enrolled in this sludy.After 24-month follow-up the 12th-and 24th-month rates of tubal patency,intrauterine pregnancy,and tubal pregnancy were evaluated.Results Inspections at the 12th and 24th month after the therapy showed that the experimental group had a significantly higher rate of complete tubal patency than the control group,while the re-occlusion rate and the half-occlusion rate in the experimental group were significantly lower than those in the control group.Inspections at the 12th and 24th month after the therapy revealed that the experimental group had a significantly higher rate of intrauterine pregnancy than the control group,and a significantly lower rate of tubal pregnancy than the control group.Conclusion The therapy involving FTR and the application of blood-activating-dispersing TCM for infertility caused by fallopian tube obstruction has favorably clinical efficacy.
