火热证的诊断量化研究

目的建立火热证中医诊断量化标准。方法通过文献出现的频率确定研究指标,在流行病学调查的基础上,应用条件概率换算方法建立症状体征赋分表,应用最大似然判别法确定量化诊断阈值,建立诊断计分表。结果按文献出现频率高低排列,前27个研究指标在火热证和健康人出现的例数比较具有显著差异;火热证诊断阈值为63;通过回顾性检验和前瞻性检验,火热证的诊断计分表的敏感度和准确度都在80%以上,阳性似然比分别为14.34和6.64。结论新建立的诊断量化标准,具有客观、准确的优点,符合临床实际。

Diagnostic and quantitative criteria of syndrome of fire-heat

Objective To establish the diagnostic criteria of syndrome of fire-heat.Methods Study indexes were determined according to the frequency of records and valued by conditional probability on the ground of epidemiological survey.Diagnositic Scores Table was established by using the maximum likelihood methods.Results It has the significant difference between the patient and healthy people in the before 27 selected indexes.The threshold value was 63.According to the retrospective test and prospective test,the sensitivity and the accuracy rating of Diagnositic Scores Table were above 80%.Its positive likelihood ratio were 14.34 and 6.64.Conclusion The diagnostic criteria of syndrome of fire-heat was not only objective and precise,but also consistent with clinical.
