| 摘 要： | 目的:本研究的目的是了解血管移植物PTFE在分流道的通畅率,分流口径对向肝血流的影响以及减压效果和并发症。方法:用内径10mm普通PTFE和带支撑环的PTFE先后为12例及8例肝硬化门脉高压症患者进行了门腔静脉间的H型分流术,其中肝功能在Child A级13例,B级5例,C级2例。平均年龄43.4岁。结果:无手术死亡。术后平均随访17.2月,脑病Ⅰ～Ⅱ级2例,无曲张静脉破裂再出血。术后门脉系统彩色多谱勒超声及血管造影检查,向肝血流量无明显影响,分流道通畅率90％,普通PTFE者血栓形成2例。结论:该技术在减低门脉压力,保证向肝血流,预防再出血等方面均取得了良好结果。其中带支撑者优于普通型。

| Abstract: | Objective: To study the patency of PTFE and its clinical results. Methods: 10mm - diameter PTFE with or without supporting rings were used in portacaval "H"type shunt operation on 12 and 8 patients respectively (Child A 13 cases, B 5 cases, C 2 cases) at our hospital during the past four years. Results: No operative mortality and rebleeding occurred but encephalopathy was observed in 2 cases over a mean follow - up period of 17. 2 monthe. Postoperative angiography and Doppler - Ultrasonic examination showed shunt patency in 90% of cases. Partial shunt thrombosis occurred in 2 cases in PTFE graft without supporting rings. Conclusion: Small -diameter portacaval shunts provide effective treatment for portal hypertension in control ling rebleeding and maitaining an adequate hepatopetal blood flow. PTFE graft using supporting rings might be more acvantageous. |
