妊娠期糖尿病母体血清尿酸水平与妊娠结局的相关性研究

目的：探讨妊娠期糖尿病孕妇孕中期血清尿酸水平对妊娠结局的影响。方法回顾性研究2013年1月至2013年12月在广州市妇女儿童医疗中心进行产检及分娩的妊娠期糖尿病孕产妇的资料，收集孕中期尿酸检测结果、妊娠并发症、妊娠结局等数据。结果孕妇检测尿酸平均孕周为21．27±5．67周，平均尿酸水平为246．15±60．17μmol／L，不同尿酸水平组孕妇发生子痫、早产比例不同，尿酸水平≥335μmol／L组发生子痫、早产的比例高于其他组（χ2值分别为20．95、14．02，均P＜0．01）。不同尿酸水平组孕妇发生羊水过多、羊水胎粪污染、胎膜早破、巨大儿、剖宫产比例无显著性差异（χ2值分别为2．14、0．97、5．57、4．12、5．25，均P＞0．05）。结论妊娠中期血清尿酸水平对妊娠期糖尿病母儿预后判断有一定的临床价值，适时采取有效的治疗措施，有利于改善妊娠结局。

Relationship between maternal serum uric acid level in gestation diabetes mellitus and pregnancy outcomes

Objective To explore the relationship between maternal serum uric acid level in gestation diabetes mellitus ( GDM ) and pregnancy outcomes .Methods Retrospective analysis was conducted on data of women with GDM who took prenatal examination and delivered in Guangzhou Women and Children ’s Medical Center in 2013.Serum uric acid level in mid-trimester, pregnancy complications and pregnancy outcomes were collected .Results The mean gestational week was 21.27 ±5.67 and the average serum uric acid ( SUA) level was 246.15 ±60.17μmol/L.The incidence of eclampsia and preterm birth was different in groups with different SUA levels , and the group with SUA≥335μmol/L had higher incidence of eclampsia and preterm birth than other groups (χ2 value was 20.95 and 14.02, respectively, both P<0.01).But there were no significant differences in incidence of polyhydramnios , meconium stained amniotic fluid, premature rupture of membranes , macrosomia, and caesarean section among groups with different SUA levels (χ2 value was 2.14, 0.97, 5.57, 4.12 and 5.25, respectively, all P>0.05).Conclusion The level of detected maternal serum uric acid in mid-trimester is valuable in predicting maternal and neonatal prognosis .Appropriate intervention in pregnancy is helpful for improving pregnancy outcomes .
