益气除湿解毒汤治疗生殖道高危型人乳头瘤病毒感染临床研究

目的:评价益气除湿解毒汤治疗生殖道高危型人乳头瘤病毒(HR-HPV)感染的临床疗效。方法:将130例HRHPV感染患者随机分为常规组和研究组,各65例。常规组给予重组人工干扰素α-2b治疗,研究组给予益气除湿解毒汤联合重组人工干扰素α-2b栓治疗,1个月经周期为1疗程,均治疗3个疗程;观察比较2组治疗前后HPV-DVA、炎症因子白细胞介素-18(IL-18)及相T淋巴细胞亚群(CD8^+、CD4^+/CD8^+)水平变化,比较2组HPV转阴率和中医证候积分情况。结果:治疗后,2组HPV DNA水平均较治疗前明显下降(P<0.05),且研究组HPV DNA水平明显低于常规组(P<0.05)。HPV转阴率研究组为76.92%,常规组为49.23%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组血清IL-18水平均较治疗前明显下降(P<0.05),且研究组IL-18水平低于常规组(P<0.05)。治疗后,研究组CD8^+较治疗前明显下降(P<0.05),CD4^+/CD8^+明显升高(P<0.05);且研究组上述指标改善均优于常规组(P<0.05)。常规组治疗前后CD8^+、CD4^+/CD8^+变化不明显(P>0.05)。治疗后,2组中医证候积分均较治疗前明显下降(P<0.05),且研究组积分明显低于常规组(P<0.05)。2组治疗过程中均未出现明显不良反应,安全性较好。结论:益气除湿解毒汤治疗HR-HPV患者,可提高患者HR-HPV转阴率,改善炎症因子水平及免疫功能,值得临床推广应用。

Clinical Study on Yiqi Chushi Jiedu Tang for Genital Tract High-risk Human Papillomavirus Infection

Objective: To observe theclinical effect of Yiqi Chushi Jiedu tang for genital tract high-risk human papillomavirus(HR-HPV) infection. Methods:A total of 130 patients with HR-HPV were randomly divided into the convention group and the study group,65 cases in each group. The convention group was treated with recombinant human interferon α-2 b suppository,while the study group was treated with Yiqi Chushi Jiedu tang combined with recombinant human interferonα-2 b suppository. Both groups were treated for three courses,one menstrual cycle being one course. Changes in levels of HPV-DVA,interleukin-18(IL-18) and T lymphocyte subsets(CD8^+ and CD4^+/CD8^+),as well as the rates of HPV negative conversion and the score of Chinese medicine syndrome were observed or compared between the two groups before and after treatment. Results: After treatment, the levels of HPV-DNA in both groups were significantly decreased when compared with those before treatment(P<0.05), and the one in the study group was significantly lower than that in the convention group(P<0.05). The rate of HPV negative conversion was 76.92% in the study group and 49.23% in the convention group,the difference being significant(P<0.05). After treatment,the levels of serum IL-18 in both groups were significantly decreased when compared with those before treatment(P<0.05), and the one in the study group was significantly lower than that in the convention group(P<0.05). After treatment, the level of CD8+ in the study group was significantly decreased when compared with that before treatment(P<0.05),and the level of CD4^+/CD8^+ was significantly increased(P<0.05);the improvement of the above indexes in the study group was superior to that in the convention group(P<0.05). There was no significant difference in the comparison of the levels of CD8^+ and CD4^+/CD8^+ in the convention group before and after treatment(P>0.05). After treatment, the scores of Chinese medicine syndrome in both groups were significantly decreased when compared with those before treatment(P<0.05), and the one in the study group was significantly lower than that in the convention group(P<0.05). No obvious adverse reactions occurred in the two groups with great safety. Conclusion: Yiqi Chushi Jiedu tang can improve the rate of HR-HPV negative conversion, the level of inflammatory factors and immune function in patients,which is worthy of clinical promotion and application.
