2型糖尿病患者血清hs-CRP与左室功能关系的研究

摘    要:【目的】探讨2型糖尿病患者血清超敏C反应蛋白(hs—CRP)水平与左室功能的关系。【方法】以本院连续诊治的218例2型糖尿病患者为研究对象,入院当天抽血测定hs—cRP的浓度;次日行心脏超声检查,评价左室功能,并测试6分钟步行试验以评估患者的运动耐量。【结果】根据hs—CRP水平分为高hPCRP组(n=164)和低hs—CRP组(n=54),心脏超声提示与低hs—CRP组相比较,高hs—CRP组左室射血分数(LVEF)(41.3±9.8%vs53.7±8.6%,P〈0.01)、短轴缩短率(FS)[(21.5±7.1)%vs(27.68±8.8)%,P〈0.01]明显减低,左室舒张末期容积(LVEDV)[(69.3±15.6)mt'vs(52.1±13.4)mL,P〈0.05]、左室心脏质量指数(LVMI)[(140.2±19.5)g/m^2vs(108.7±18.3g/m^2),P〈0.01]明显增加;高hs—CRP组的6分钟步行距离显著低于低hs—CRP组[(432.7±35.7)mvs(586.6±46.3)m,P〈0.01]。【结论】血hs—cRP水平可作为2型糖尿病患者心功能不全的参考指标,hs—CRP水平愈高,左室功能越差,运动耐量越低。

Relationship Between Serum High Sensitivity C-reactive Protein Level and Left Ventricular Function in Type 2 Diabetes Mellitus Patients

Abstract:[Objective]To explore the relationship of high sensitivity C-reactive protein(hs-CRP) level and left ventricular function in type 2 diabetes mellitus. [Methods]A total of 218 patients with type 2 diabetes mellitus were consecutively included into the study. At day of admission, hs-CRP concentration was measured. And left ventricular function was evaluated by echocardiogram at next day. Six-minute walking test was performed to assess exercise capacity. [Results]All patients were divided into high hs-CRP( n=164)group and low hs-CRP( n=54)group. LVEF(41.3±9.8% vs 53.7±8. 6%, P〈0.01)and FS(21.5±7.1% vs 27.68±8.80//00, P〈0.01) in high hs-CRP group were significantly lower than those in low hs-CRP group while LVEDV(69.3±15.6 mL vs 52.1±13.4 mL, P〈0.05) and LVMI (140.2±19.5 g/m^2 vs 108.7±18.3 g/m^2, P〈0.01) were significantly higher. Distance of six-minute walking test in high hs-CRP group was significantly lower than that in low hs-CRP group. [Conclusion] Serum hs-CRP level can serve as a reliable biomarker for cardiac dysfunction in patients with type 2 diabetes mellitus. The higher hs-CRP level suggests lower left ventricular function and exercise capacity.
