定量组织速度成像技术评价肥厚型心肌病患者左室纵向收缩功能

摘    要:目的明确定量组织速度成像技术(QTVI)评价肥厚型心肌病(HCM)左室纵向收缩功能的价值.方法应用QTVI离线分析31例HCM病人和20例正常对照组左室长轴方向各心肌节段的速度曲线,测量收缩峰值速度和峰值时间,计算同一水平不同节段的平均峰值速度(Vs)和峰值时间(Ts).结果 HCM组二尖瓣环、左室基底部、中部及近心尖部各水平Vs均降低,Ts均延长,与对照组相比差异均极具有显著性(P<0.01).结论 HCM病人尽管左室射血分数正常,但左室长轴方向各心肌节段收缩功能受损,QTVI能定量评价HCM患者左室纵向收缩功能.

Abstract:Objective To determine the value of the quantitative tissue velocity imaging (QTVI) for the evaluation of the longitudial left ventricular systolic function in patients with hypertrophic cardiomyopathy (HCM). Methods Off line profiles of the left ventricular regional systolic velocity along long axis in 31 HCM patients and 20 healthy subjects were analyzed by QTVI. The regional myocardical systolic peak velocities and times from the electrocardiographic Q wave to the peak of the systolic wave of each segment were measured. The mean peak velocities (Vs) and times (Ts) of different segments from the same level were calculated. Results Vs was lower and Ts was longer at each level including mitral annular, basal, middle and apical segments in the HCM patients than that in the healthy subjects ( P <0.01). Conclusion The left ventricular longitudinal myocardial contractility is impaired although left ventricular ejection fraction is normal in HCM group, and these results suggest that QTVI be used to assess left ventricular longitudinal systolic function in patient with HCM.
