彩色多普勒超声对多发性大动脉炎的诊断与分析

目的应用彩色多普勒超声诊断多发性大动脉炎引起的外周动脉血管及心脏病变的超声表现进行分析。方法彩色多普勒超声诊断仪，常规检查外周血管及心脏。结果5例患中，2例头臂动脉型，其中1例合并心脏病变；1例腹主动脉型；1例股总动脉型；1例。肾动脉型。超声表现为病变血管内中膜增厚不均匀，管壁不同程度的狭窄，其中1例左房、左室扩大伴左心功能减低(EF42％)。结论大动脉炎累及最多的血管为锁骨下动脉及颈总动脉，其次为腹主动脉、肾动脉、股动脉，同时可累及心脏。

Diagnosis and Analysis of Color Doppler Flow Imaging in Multiple Aorto-arteritis

Objective The misdiagnosis and missed d iagnosis may easily occur because of the relative low incidence rate and atypica l clinical features of multiple aorto-arteritis.The morpology of color Doppler flow imaging (CDFI) was investigated in the diagnosis of multiple aorto-arteri tis in patients with peripheral arterial and cardiac lesions. Methods Hitachi EUB-6000 ultrasonic device with a trandu cer of frequency 2~13 MHz was applied.The conventional lesions of peripheral ar teries and heart were scanned. Results In 5 cases with mutiple arteritis,branchiocephali c artery were in 2 cases,one of whom with cardiac disease;abdominal aort(1 case) ;common femoral artery and its branches (1 cases);renal artery (1 cases).The ar terial intima-media thickness (IMT)was nonuniform and the luminal stenosis was in various degrees.One case was with large atrium,large ventricule with the dec reased left ventricular function (EF,42%). Conclusions The first invaded arteries in multiple aorto -arteritis were subclavicular and common carotid arteries and the second ones w ere abdominal aorta,renal artery,femoral artery and cardiac invasion.
