影响子宫内膜癌复发的高危因素分析

目的回顾性分析总结影响子宫内膜癌复发的高危因素。方法对2000年12月至2004年12月收治的189例子宫内膜癌病例进行回顾性分析，进一步探讨病理类型、组织学分级、肌层浸润深度、淋巴结转移、手术病理分期、合并症与复发之间的关系。结果手术病理分期，不同病理类型、组织学分级，有无肌层浸润和淋巴结转移中患者的缓解曲线比较，结果均P〈0．05，差异有统计学意义；有无合并糖尿病、高血压或肥胖患者的缓解曲线比较，结果均P〉0．05，差异无统计学意义。COX回归多因素分析，肌层浸润深度及术后病理类型对复发的影响有统计学差异，P〈0．05。结论特殊病理类型、组织学低分化、深肌层浸润、淋巴结转移、期别晚是复发的高危因素。

A clinical analysis of endometrial carcinoma

Objective To analyze high risk factors for prognosis of endometrial carcinoma. Methods To summarize the influence of pathologic type, histologic grade, myometrial invasion depth, lymph node metastasis, surgical-pathologic stage and complications on prognosis of 189 cases with endometrial carcinoma from Dec. 2000 to Dec. 2004. Results The comparisons of the alleviate curves in different surgical-pathologic stages, pathologic types , histologic grades, different depth of myometrial invasion , and wether with lymph nodemetastasis were both P 〈 0. 05, they indicated the differences were statistically significant. The comparisons of the alleviate curves in wether with diabetes, hypertension or fat were both P 〉 0. 05, they were not statistically different. By multivariate analysis, depth of myometrial invasion and special pathologic type were independent high risk factors for the recurrence （P 〈 0. 05）. Conclusion Special pathologic types, high histological grade, deep myometrial invasion, lymph node metastasis, late-stages are high risk factors of recurrence.
