西酞普兰治疗脑卒中后抑郁对照研究的Meta分析

目的探讨西酞普兰治疗脑卒中后抑郁的疗效及不良反应的差异。方法用Meta分析对13项西酞普兰与其他抗抑郁剂治疗脑卒中后抑郁对照研究的文章进行再分析，评价其合并效应量大小和综合显著性检验。结果西酞普兰治疗前后的自身对照，合并效应量d=-5．84，95％CI（-7．39，-4．29），综合显著性检验（χ^2=54．46，P〈0．01），提示西酞普兰治疗脑卒中后抑郁前后症状学变化有非常显著性差异，效应极强；西酞普兰与对照药在第2周末和治疗结束后的组间比较，分别为d=-1．15，95％CI（-1．66，-0．63）,χ^2=19．10，P〈0．01；d=-0．45，95％CI（-0．75，-0．16），χ^2=8．94，P〈0．01，提示西酞普兰组疗效好于对照组，且起效快；西酞普兰的不良反应显著少于对照药物。结论西酞普兰疗效优于对照药物，不良反应少。

Meta-analysis of comparative on citalopram in the treatment of post-stroke depression

Objectlve To study the difference of efficacy and side effect of citalopram in the treatment of post - stroke depression. Method 13 comparative studies of citalopram and other antidepressants in the treatment of post - stroke depression were analyzed by Meta - analysis. Results The significant difference in change of symptom after and before treatment by citalopram was found, d = - 5.84,95 % CI（ - 7.39, - 4. 29） ,χ^2 = 54. 46, P 〈 0. 01. Comparison between citalopram and the controlled antidepressants at 2 weeks treatment and at tht end of each trial showed that d = 1.15,95% CI（ - 1.66, - 0. 63 ） ,χ^2 = 19. 10, P 〈 0. 01 ; d = - 0. 45,95% CI（ - 0. 75,0. 16） ,χ^2 = 8.94, P 〈 0. 01. Which revealed that citalopram difference from other antidepressants is in side effects, but not in therapeutic effects. Conclusion Citalopram has significant curative effecacy than the controlled antidepressants, milder adverse reactions and higher safety in the treatment of post - stroke depression.
