体外循环对血浆IL-8及中性粒细胞表面IL-8RA表达的影响

摘    要:目的 研究体外循环(CPB)期间病人血浆IL-8及中性粒细胞(PMN)表面IL-8RA表达量的变化。方法 以临床先心房室缺病人为研究对象,分别在麻醉诱导后(A)、CPB后30min(B)、CPB结束后即刻(C)采集病人外周血,测定白细胞数、血浆IL-8浓度(ELISA法)及PMN表面IL-8RA表达量(间接免疫荧光流式细胞法)。结果 B点白细胞计数显著低于A、C点(P<0.05),而B、C两时相点的血浆IL-8浓度和外周血PMN表面IL-8RA表达量均非常显著地高于A点(P<0.01),且C点值显著高于B点(P<0.05)。结论 CPB期间及CPB后血浆IL-8浓度和外周血PMN表面IL-8RA表达增加,可能是CPB导致PMN激活和转流后病人炎性损伤的机制之一。

Abstract:Objective To investigate the effects of cardiopulmonary bypass (CPB) on the plasma IL 8 concentration and IL 8RA expression on the periperal polymorphonuclear (PMN) cell surface in clinical patients. Methods Patients with atrial septal defect (ASD) or ventricular septal defect (VSD) were chosen in the study and 3 time points were selected, i.e, immediately after anesthetic induction(A), 30 min after CPB(B) and at the end of CPB (C). Peripheral venous blood was collected and the following indices were analyzed: leukocytes count, plasma IL 8 concentration and peripheral PMN surface IL-8RA expression. Results Leukocytes counts at time B were significantly lower than both time A and time C ( P <0.05), while both IL 8 concentration and IL 8RA expression on PMN at time B and C were significantly higher than time A ( P <0.01). Besides, all values at time C were significantly higher than those of time B ( P <0.05). Conclusion Plasma IL 8 concentration and PMN surface IL 8RA expression are elevated during and after CPB process, which may contribute to the activation of neutrophils and be regarded as an important reason of post pump inflammatory injuries.
