NT-proBNP动态变化对充血性心力衰竭患者近期预后的评估价值

目的探讨血浆氨基末端脑钠肽前体(NT-proBNP)的动态变化对充血性心力衰竭患者住院期间预后评估的价值。方法选取45例NYHA分级III-IV级的充血性心力衰竭患者,分别检测急诊入院时即刻、入院后24 h和72 h测量的血浆NT-proBNP,并计算72 h内血浆NT-proBNP水平变化率(△NT-proBNP),根据72 h△NT-proBNP结果分为A组(△NT-proB-NP%≥30%)、B组(△NT-proBNP%<30%),观察两组患者住院期间不良心脏事件发生率、病死率及住院时间。结果 B组患者住院期间不良心脏事件和心源性死亡率均高于A组患者,住院时间明显长于A组患者(P<0.05)。结论 NT-proBNP的动态变化可以较客观地评估充血性心力衰竭患者病情的近期预后。

Value of dynamic observation of plasma NT-proBNP levels in the short-term prognosis in the hospitalized patients with congestive heart failure

Objective To evaluate the value of dynamic observation NT-proBNP in the short-term prognosis of patients with congestive heart failure(CHF).Methods Plasma NT-proBNP levels were measured on admission,24 and 72 hours after hospitalization in 45 consecutive patients with NYHAIII-IV.The decreasing rate of plasma NT-proBNP levels(△NT-proBNP%) within 72 hours was also calculated.The patients were divided into two groups: A group(△NT-proBNP%>30%) and B group(△NT-proBNP%<30%).The incidence of adverse cardiac events,hospitalization mortality and length of hospitalization were observed.Results During the period of observation,the incidence of adverse cardiac events,hospitalization mortality and length of hospitalization in group B within 72 hours were higher than those of patients in group A.Conclusion Consecutive monitoring of plasma NT-proBNP levels can predict the short-term prognosis of patients with congestive heart failure.
