腔镜辅助保留回盲瓣结肠次全切除治疗顽固性慢性运输型便秘

目的探讨腹腔镜辅助保留回盲瓣的结肠次全切除术治疗顽固性慢性运输型便秘。方法将需手术治疗顽固性慢性运输型便秘的31例患者分成两组，实验组行经腹腔镜保留回盲瓣的结肠次全切除术，对照组施行开腹保留回盲瓣的结肠次全切除术，比较两组的手术时问、切口长度、出血量、术后疼痛、肠道功能恢复时间、治疗效果及切口愈合。结果两组比较，手术时间、治疗效果差异无统计学意义（P〉0．05）；与对照组比实验组平均切口长度、术中出血量、镇痛药用药次数、肠道恢复时间均显著减少（P〈0．05），切口一期愈合率显著提高（P〈0．05）。结论腹腔镜辅助保留回盲瓣的结肠次全切除术治疗顽固性慢性运输型便秘创伤小、恢复快、效果好，是一种值得推广应用的术式。

The effect of 16 cases of chronic obstipation treated by subtotal colectomy through laparoscope.

Objective To investigate the effect of chronic obstipation treated by subtotal colectomy through laparoseope.Methods The 31 patients with ehronieial obstipation were randomly divided into 2 groups, the experimental group （16 cases）were treated with subtotal colectomy by laparoscope, the control group（ 15 cases） were treated with subtotal coleetomy by operation, the duration of operation,the length of operation nick,the amount of bleeding in operation, pain after operation, coincidence time of the function of intestinal tract and curative effect between EG and CG were compared. Results There were no significant difference between the two groups in the operation time,curative effect（ P 〉 0.05） ,but the experimental group was better than the control group in the length of operation nick, the amount of bleeding in operation, pain after operation ,coincidence time of the function of intestinal tract （ P 〈 0.05 ）. Conclusion The method of theraping of chronlcity obstipation with subtotal ectomy by laparoscope is better than that with subtotal ectomy by operation, the former is worth to be generalized.
