彩色多普勒超声在宫外孕中的诊断价值

目的：探讨彩色多普勒超声在宫外孕中的诊断价值。方法：选取我院自2006年1月至2008年1月间术前经彩色多普勒超声诊断的宫外孕患者90例,总结分析超声诊断符合率、声像特征等。结果：本组90例患者后经手术、病理证实为宫外孕者86例,准确率为95.56%。本组64例（66.67%）患者子宫轻度增大。输卵管妊囊型27例（30.00%）,表现为较完整胎囊强回声光环,部分见胚芽;输卵管包块型52例（57.78%）,表现为边界不规则的包块回声,内部回声不均匀,胎囊显示不清楚;输卵管妊娠破裂出血时盆腔可见少或中量积液等。CDFI检查显示在囊实肿块内或周边见到点状或条状彩色血流信号,呈动脉样血流频谱,包络线不清楚,呈单相或双向低阻力,Vs为（50.05±5.02）cm/s,Vd为（23.20±4.45）cm/s,RI为（0.43±0.02）。结论：彩色多普勒超声在诊断宫外孕中具有其他检查方式无法比拟的优势,值得临床应用。

Diagnostic Value of Color Doppler for Patients with Ectopic Pregnancy

Objective： To investigate the diagnostic values of color doppler on patients with ectopic pregnancy. Method： 90 cases with ectopic pregnancys who received color doppler ultrasonic diagnosis in our hospital from January in 2006 to January in 2008 were selected and the coincidence of ultrasonic diagnosis and feature of acoustic image were analyzed. Result： 86 cases in 90 patients were confirmed to be ectopic pregnancy by operation and pathology, and the coincidence of ultrasonic diagnosis was 95.56%. The uterus of 64 cases（66.67% ）became a little augmentation. 27 cases （30.00% ）were with type of uterine tube pregnant - scrotiform, and the feature of acoustic image were the dense echo of whole gestational sac in accompaniments, few with plumules. The 52 cases （ 57.78% ） were with type of uterine tube clumps, and the feature of acoustic image showed clump echo were irregularity boundary, internal echo were uneven, gestational sac were not clear and there were a little or a few of fluidify when tubal pregnancys were ruptured. The CDFI showed that the punctiform or strip color blood flow signal were found, which blood current spectrum were like the artery, envelope curves were not clear, monophase or bilateral low resistance. The Vs were （ 50.05±5.02） cm/s, Vd were （23.20±4.45 ） cm/s and RI were （0.43±0.02 ）. Conclusion ： There are more merits in color doppler than others, so the color doppler is worth being utilized widly.
