正常人甲状腺剪切波速度参考值范围的初步研究

摘    要:目的探讨声触诊组织量化(VTQ)技术检测甲状腺硬度的可行性,建立正常人甲状腺剪切波速度参考值范围。方法应用超声诊断仪内置的VTQ技术对90名健康体检者甲状腺双侧叶进行横切面及纵切面VTQ检测,获得甲状腺的剪切波速度。前20名志愿者分别接受两名经过VTQ技术培训的超声主治医师进行甲状腺相同部位VTQ的检测。正常甲状腺剪切波速度参考值按(?)±1.96s计算。结果 (1)甲状腺双侧叶横切面与纵切面剪切波速度差异无统计学意义(P〉0.05);甲状腺左侧叶与右侧叶剪切波速度差异无统计学意义(P〉0.05)。(2)老年组甲状腺剪切波速度与青年组及中年组差异均有统计学意义(均P〈0.05),而青年组与中年组差异无统计学意义(P〉0.05)。不同性别组甲状腺剪切波速度差异有统计学意义(P〈0.05);甲状腺不同部位剪切波速度组内相关系数为0.85.(3)不同操作者对同一组研究对象进行甲状腺VTQ检测的测值差异无统计学意义(P〉0.05)。结论 VTQ技术可重复性及稳定性好,应用该技术进行甲状腺硬度评价是可行的。

Abstract:Objective To explore the feasibility of detecting thyroid stiffiness with virtual touch tissue quantification(VTQ) technique and establish reference value of thyroid shear-wave velocity in healthy people.Methods Ninty healthy volunteers from physical examination center underwent VTQ detection to obtain the shear-wave velocities of thyroid.Thyroid shear-wave velocities were measured with ultrasound system.Twenty subjects were performed twice by two sonographers.Results(1 ) There was no significant difference of thyroid shear-wave speed between the transverse plane and the longitudinal plane.There was no significant difference of thyroid shear-wave speed between the right thyroid lobe and the left thyroid lobe.(2) There was no significant difference of shear-wave speed between the youth group and the middle -aged group,while there was significant difference between the elderly group and the other two groups (均P〈0.05).Shear-wave speed in different gender had statistically significance(P〈0.05).The ICC of measurement value of all parts in health thyroid was 0.85.(3) There was no statistically significance of thyroid shear-wave value in healthy people undergoing different operaions.Conclusion VTQ technique has good repeatability and stability.Assessment of thyroid stiffness using this technology is feasible.
