摘    要:目的探讨肠内营养支持对老年慢性阻塞性肺疾病急性加重期(AECOPD)并呼吸衰竭患者的心肺功能及营养免疫状况等的影响。方法 127例机械通气AECOPD并呼吸衰竭的老年患者随机分为两组,肠内营养组68例和对照组59例(常规饮食),两组患者均给予抗感染、相关对症治疗,营养支持治疗期为2周。结果肠内营养组治疗后,营养指标较治疗前及对照组明显提高(P0.05)。肠内营养组治疗后心肺功能及免疫功能指标均较治疗前及对照组显著改善,hs-CRP和TNF-α等炎症指标提前降低(P0.05)。结论老年AECOPD合并呼吸衰竭患者,在常规治疗的同时给予早期肠内营养治疗,可以改善患者营养状况、心肺功能、免疫功能。

Effect of enteral nutrition on cardio-pulmonary function and immunity function in elderly patients with AE-COPD complicated with respiratory failure
Abstract:Objective To investigate the effect of enteral nutrition on cardio-pulmonary function and immu-nity function in elderly patients with AECOPD complicated with respiratory failure who had received non-invasive pos-itive pressure ventilation ( NIPPV) . Methods 127 elderly patients with AECOPD and respiratory failure who had received NIPPV were randomly divided into the enteral nutrition group (n=68) and the control group (n=59, regu-lar diet) . All patients were given antibiotics and other symptomatic treatment, and nutritional support treatment peri-od was two weeks. Results After the treatment, the nutrition indicators were significantly higher in the treatment group than in the control group (P<0. 05), and the cardio-pulmonary and immunity function in the enteral nutrition group were better than in the control group (P<0. 05). After the treatment, the levels of hs-CRP and TNF-αpresen-ted earlier downward trend in the treatment group while compared with those in the control group (P<0. 05). Con-clusion The enteral nutrition combined with conventional treatment can improve the nutrition status, cardio-pulmo-nary function and immunity function in elderly patients with AECOPD complicated with respiratory failure.
