医护患一体化教育对老年2型糖尿病病人自我效能及健康知识掌握的影响

摘    要:目的 探讨医护患一体化的健康教育护理模式对老年糖尿病病人自我效能及健康知识掌握的影响。方法 选取中国医科大学附属盛京医院老年病科2014年9月至2015年5月住院的老年糖尿病病人100例,按随机数字表法分为两组,对照组50例给予常规护理干预,观察组50例给予医护患一体化的健康教育护理,比较干预前后自我效能感评分和健康知识掌握情况。结果 两组干预前自我效能感评分差异无统计学意义(t=1.056,P=0.294);干预1年后均有提高,观察组(35.30±3.28)分显著优于对照组(27.12±1.75)分(t=15.560,P<0.001)。观察组健康知识掌握情况显著优于对照组,差异有统计学意义(P<0.001)。结论 在老年糖尿病病人中实施医护患一体化的健康教育护理,可显著提高病人自我管理能力及健康知识的掌握水平,对改善老年糖尿病病人的预后具有非常重要的临床意义。

Abstract:Objective To explore the effect of doctor-nurse-patient integration model in self-management education on the glucose metabolism and self-efficacy of elderly patients with diabetes mellitus.Methods A total of 100 elderly patients with diabetes mellitus were enrolled in this study,which were admitted to Shengjing Hospital of China Medical University from September 2014 to May 2015.The patients were randomly divided into control group (n=50) and observation group (n=50).Patients in control group received routine nursing intervention for one year,while the observation group received doctor-nurse-patient integration model in self-management education on the basis of conventional nursing for one year.The self-efficacy and mastery of health knowledge of patients in two groups were analyzed before and after intervention.Results There was no significant difference in self-efficacy scores between the two groups before intervention(t=1.056,P=0.294).After intervention,the self-efficacy scores in both groups were elevated,while the score in the observation group was significantly higher than that of the control group (35.30±3.28 vs.27.12±1.75,t=15.560,P<0.001).In addition,the mastery degree on health education of cases in the observation group was better than in control group (P<0.001).Conclusion doctor-nurse-patient integration model in self-management education for elderly patients with diabetes mellitus is effective.It can not only improve patients'' self-management ability,but also increase patients'' awareness about health knowledge,and has very important clinical significance for prognosis.
