摘    要:目的 观察关节镜下半月板成形术配合术后中药活血利水汤内服治疗退行性半月板损伤的临床疗效;方法 将退行性半月板损伤患者60例随机分成对照组和观察组。两组均行关节镜下半月板修整成形术,观察组术后予以活血利水汤内服。术后随访半年,分别对两组的治疗有效率、膝关节疼痛视觉模拟评分(Visual analogue scale,VAS),膝关节西安大略和麦克马斯特大学骨关节炎指数(Western Ontario McMasterUniversity Osteoarthritis Index,WOMAC)、LequesneMG、Lysholm(The Lysholm Knee Scale)、JOA(日本矫形外科学会,Japanese Orthopaedic Association)功能评分、术中Outbridge评分,结合术后生活质量(SF-36)评分进行观察;结果 两组各观察指标均较术前明显改善,有统计学意义(P < 0.05),两组术后比较,观察组在膝关节VAS疼痛评分,膝关节WOMAC、LequesneMG、Lysholm、JOA功能评分以及术后生活质量(SF-36)评分均较对照组改善明显,有统计学意义(P < 0.05);结论 半月板成形术是治疗退行性半月板损伤的有效方法,活血利水汤能改善术后膝关节肿痛、促进膝关节术后功能恢复。

Abstract:Objective To observe clinical efficacy of meniscus plasty under arthroscopy and oral Huoxue Lishui Decoction after the operation in the treatment of degenerative meniscus injury. Methods Sixty patients with degenerative meniscus injury were randomly divided into observation group and control group. The 2 groups were treated by the meniscus plasty under arthroscopy. The observation group was treated with HuoxueLishui Decoction orally. Effective rate, VAS score of pain in the knee joint, WOMAC, Lequesne MG, Lysholm and JOA scores of the knee joint, Outbridge score during the operation and quality of life after the operation (SF-36) were observed in the 2 groups, and all the patients were followed upfor half a year. Results The indexes in the 2 groups were markedly improved in comparison to those before the operation, and the differences were statistically significant (P < 0.01 or P < 0.05). After the operation, in the observation group, the VAS pain score, the WOMAC, Lequesne MG, Lysholm and JOA socres of the knee joint, and SF-36 score were improved significantly compared to those in the control group. And there was statistical difference (P < 0.05). Conclusion: Meniscus plasty is an effective method for treating degenerative meniscus injury. Huoxue Lishui Decoction can improve the swelling and pain of knee joint after the operation and promote the recovery of knee joint function after the operation, with significant efficacy.
