高龄高危前列腺增生症经尿道电汽化术结合电切术治疗体会

目的 总结高龄高危良性前列腺增生症（BPH）患者经尿道电汽化术结合电切术的临床治疗经验。方法 联合应用经尿道前列腺电汽化术（TUVP）和经尿道电切术（TURP）治疗高危、高龄（年龄〉70岁）BPH患者82例。结果 所有患者术后随访1个月-1年，排尿功能恢复满意。国际前列腺症状评分（IPSS）、生活质量评分（QOS）、最大尿流率（Qmax）等方面有显著改善（P〈0．01）。且无严重并发症出现。结论 年龄在70岁以上伴有心、肺、脑、肾等重要脏器合并症的高危BPH患者，经TUVP联合TURP治疗，具有出血少、安全性高、并发症少、疗效确切等优点，是高龄高危BPH患者最佳治疗方法。

Treatment of senior and high-risk benign prostatic hyperplasia by transurethral electrovaporization and resection of the prostate

Objective] To summarize the clinical therapeutic experience of transurethral electrovaporization and resection of the prostate in the treatment of senior and high-risk BPH. Methods] Transurethral electrovaporization of the prostate (TUVP) and transurethral resection of the prostate (TURP) were used to treat 82 cases of senior and high-risk benign prostatic hyperplasia(BPH). Results] All patients were followed up for one month to one year postoperatively with satisfatory function of urination. It was found the international prostate sympton score (IPPS), the associated quality-of-life score (QOS ), the maximum urinary flow rate (Qmax) were significantly improved (P <0.01) without serious complications of occurrence. Conslusion] Patients over 70 years old suffered from highly risk BPH with severe complications in heart, lungs, brain and kidneys were treated by TUVP combined with TURP with less bleeding, more safety, less complications and definite effect. TUVP combined with TURP is the best choice of treatment for senior and high-risk BPH.
