彩色多普勒超声诊断妊娠合并卵巢扭转的临床价值

目的 评估彩色多普勒超声对妊娠合并卵巢扭转（ovarian torsion,OT）的诊断价值.方法 对11例中晚期妊娠患者因腹部疼痛进行彩色多普勒超声检查,结果与手术、腹腔镜及病理结果对照.结果 患者患侧附件区可见异常包块回声,内部呈较低回声.7例包块边缘蒂部可见螺旋状扭转.彩色多普勒血流显像（CDFI）及彩色多普勒能量图（CDE）显示10例包块内及蒂部无血流信号;1例包块内见散在极少量星点状血流信号,蒂部见斑点状血流信号,阻力指数0.3～0.4.超声检查定位准确率100%,定性正确率100%.结论 彩色多普勒超声诊断妊娠合并OT有较高的敏感性和特异性,具有较高临床应用价值.

Assessment of the diagnosis of ovarian torsion in pregnancy with ultrasonography and Doppler

Objective To assess the clinical value of the diagnoses of ovarian torsion in pregnancy with ultrasonography and Doppler. Methods 11 cases of the second and third trimester pregnancies complained pelvic or abdominal pain concornitanting with nausea or vomiting referred to hospital undertook workup of Ultrasonography and Doppler. The outcome was checked with the findings of surgery or laparotorny. Results There were various shape and size masses in the places the ovaries had had been. Checked with CDFI and CDE, there were no signal in the mass and pedic!es in 10 masses of cases ; There were paucity signal in the mass and pedicle in one case. Conclusion It is valuable to diagnose ovarian torsion in pregnancy with ultrasonography and Doppler.
