老年高血压病血压类型及对左室功能影响的研究

目的 通过对200例老年高血压病患者血压及左室功能测定，分析老年高血压病血压类型和对左室功能的影响。方法 随机选择健康老年人50例，老年高血压病患者200例，根据血压分为单纯收缩性高血压（ISH），单纯舒张性高血压（IDH）和混合性高血压（MH）。分别测左室射血分数（EF）、每搏输出量（SV）、每分输出量（CO）、心脏指数（CI）以及A峰值速度（Peak A），E峰值速度（Peak A)，E峰值速度(Peak E)，峰值速度比值（A／E），峰值充盈（PER），充盈分数（SF）和心率（HR）。结果 老年高血压病血压类型ISH为常见，占675，LDH占8％，MH占25％，3组间差异显著（P＜0.01）。反映左室收缩功能的EF、CI、SV下降，以ISH和MH组明显；反映左室舒张功能的E峰、PER下降，以ISH组为著，3反映左室收缩功能的EF、CI、SV下降，以ISH和MH组明显；反映左室舒张功能的E峰、PER下降，以ISH组为著，3组间差异显著（P＜0.01）。结论 老年高血压病血压以ISH为主，IDH最少，与健康老人比较，左室功能明显下降，尤以ISH组显著。

Study on type of blood pressure and left vertricular per formance in senile patients with essential hypertension

Objective To study on type of blood pressure and left vertricular performance in 200 senile patients with essential hypertension.Methods 50 normal and 200 patients were divided into ISH,IDH and MH according to blood pressure.The left ventricular of EF,SV,CO,CI,peak A,peak E,A/E,PER,SF,HR were meassured.Results The ISH occured in 67%.IDH occured in 8%.MH occured in 25%.There were significant differences among the three groups ( P <0.01).Left ventricular function were altered significantly,the systolic functions of EF,CI,SV were decrease,especially ISH and MH,the diatolic functions of peak E,PER were decrease,especially ISH.There were significant differences among the three groups ( P <0.01).Conclusion The ISH is the most common and the IDH is the least disease in senile patients with essential hypertension.The left ventricular functions in contrast to the normal individuals are decrease.The decrease of function is most marked in ISH.
