血清25-羟维生素 D3水平与2型糖尿病患者大血管病变的相关性研究

摘    要:目的:探讨血清25-羟维生素 D3(25-hydroxyvitamin D3,25(OH)D3)水平与2型糖尿病(type 2 diabetes mellitus, T2DM)大血管病变(macro-vascular complications,MVC)相关性。方法:选取 T2DM 患者176例,按照有无 MVC 分为单纯T2DM组(108例)和 T2DM合并 MVC 组(68例)。统计两组患者临床资料,并检测常规生化指标、血清25(OH)D3水平;分析T2DM患者发生 MVC 的独立危险因素。结果:T2DM合并 MVC 组患者年龄、病程、吸烟率、LDL-C 及 hsCRP 高于单纯 T2DM组,25(OH)D3水平低于单纯 T2DM组,差异具有统计学意义(P <0.05、P <0.01);多因素 Logistic 分析显示糖尿病病程、年龄、吸烟史、LDL-C 为 MVC 的危险因素,25(OH)D3为保护性因素;T2DM 患者血25(OH)D3水平与年龄、糖尿病病程呈负相关。结论:血清25(OH)D3水平与 MVC 发生呈负相关,25(OH)D3水平降低可能会增加 T2DM患者 MVC 的风险。

Study on relation between serum 25(OH)D3 level and MVC in T2DM pa-tients
Abstract:Objective:To explore the relevance between serum 25-hydroxyvitamin D3 (25 (OH)D3 )level and macro-vascular complications (MVC)in patients with type 2 diabetes mellitus (T2DM).Methods:A total of 176 T2DM patients were selected and di-vided into single T2DM group (108 cases)and combination group of T2DM and MVC (68 cases)according to the presence of MVC. Clinical data of both groups were collected to detect the routine biochemical indexes and serum 25(OH)D3 level and to analyze the in-dependent risk factor of MVC in T2DM patients.Results:Combination group of T2DM and MVC was evidently higher in ages,disease courses,rate of smoking,low density lipoprotein cholesterol (LDL-C),high-sensitivity C reactive protein (hsCRP)markedly lower in 25(OH)D3 l evel than single T2DM group,and all differences were significant (P <0.05,P <0.01 ).Multivariate Logistic analysis showed that disease course,ages,smoking history and LDL-C were risk factors of MVC while 25(OH)D3 was protective factor,and 25 (OH)D3 level was in negative association with ages and disease course of T2DM.Conclusion:Serum 25(OH)D3 level is closely asso-ciated with the development of MVC,and its level decrease may increase the risk of MVC in patients with T2DM.
