摘 要: | 目的:研究^99mTc—MDP骨扫描和肿瘤标记物在非小细胞肺癌分期诊断中的价值。方法:对156例初诊非小细胞肺癌患者行99mTc—MDP骨扫描,并收集肺癌患者和30例正常人的.血清标本,用化学发光法检测CEA、CA125、NSE和CYFRA21—1四项指标。结果:病人CEA、CA125、NSE和CYFRA21—1高于正常人(P〈0.01),并且随着期别进展,肿瘤标记物水平逐步升高。^99mTc—MDP骨扫描明确骨转移的占25.64%(40/156),Ⅳ期81例患者48.12%(39/81)发生骨转移。结论:^99mTc—MDP核素骨扫描可作为肺癌分期检查的筛查手段;随着肺癌病程的进展,肿瘤标记物CEA、CA125、NSE和CYFRA21—1的水平逐渐上升,^99mTc—MDP骨扫描联合肿瘤标记物在非小细胞肺癌分期诊断中具有重要的价值。

Abstract: | Objective: To study the value of combination 99mTc - MDP bone scanning with serum tumor marker measurement in the stages of patients with non - small lung cancer. Methods: 99mTc - MDP whole - body bone scan- ning was performed in 156 patients with non - small lung cancer, the serum samples were collected in 156 patients and 30 comparison controls. CEA, CA125, NSE and CYFRA21 -1 were measured by chemiluminescence technique. Results: In 156 patients with non- small lung cancer, Ⅰ stage patients were 6. 41% ( 10/156 ) , Ⅱ stage patients 5.77% (9/156), Ⅲ stage patients 15.38% (24/156), Ⅳ stage patients 51.92% (81/156) ,unknowable stage patients 20.51% (32/156). The average value of CEA and CYFRA21 - 1 in I stage patients was higher than the control group( P 〈 0.05 ). CEA, CA125, NSE and CYFRA21 -1 inⅡ stage patients was higher than the control group (P 〈 0.01 ). Following the stage advancement the revel of these serum tumor marker was higher and higher. The positive ratio of patietens with osseous metastasis was confirmed by 99mTc - MDP whole - body bone scanning was 25.64% (40/156), 48.12% (39/82) patients osseous metastasized in 82 IV stage patients. Conclusion: 99mTc- MDP bone scanning was considerable in the stages of lung cancer, following the stage advancement CEA, CA125, NSE and CY- FRA21 - 1 was higher and higher. Combination 99mTc - MDP bone scanning with serum tumor marker measurement are valuable to the stage diagnosis of non - small lung cancer. |
