早发型重度子痫前期患者及新生儿甲状腺相关激素水平的变化研究

摘    要:目的:研究早发型重度子痫前期孕妇的甲状腺功能变化及对胎儿发育的影响。方法:采用病例对照研究方法,按子痫前期患者孕周分组,检测各组患者及新生儿促甲状腺激素(TSH)、总甲状腺素(TT4)、游离甲状腺素(FT4)、总三碘甲腺原氨酸(TT3)、游离三碘甲腺原氨酸(FT3)水平,入院时采用B超检测胎儿发育状况,进行胎儿生物物理4项评分(BPS)。结果:各组患者TSH水平均有显著性升高,胎儿BPS分值均显著性降低,孕34~36周子痫前期患者FT4水平降低。孕28~33周子痫前期患者各组新生儿各项激素水平均低于对照组。结论:子痫前期甲状腺功能异常,对胎儿发育产生不利影响,其甲状腺功能的改变与孕周存在一定程度的相关性。

Abstract:Objective:To study maternal and neonatal thyroid hormonal status in early-onset severe pre-eclampsia.Method:Maternal and neonatal thyroid hormonal status including thyroid-stimulating hormone(TSH),total thyroxin(TT4),free thyroxin(FT4),total triiodothyronine(FT3),free triiodothyronine(FT3) were tested and Biophysical Profile Score(BPS) was performed at the time of admission.Results:TSH level was significantly increased in 3 groups with BPS significantly decreased.Neonatal thyroid hormonal level was decreased in patients of gestational age 28-33 weeks.Conclusion:Thyroid hormonal status of pregnancy with pre-eclampsia is abnormal and it has adverse effects on the development of the fetus.
