创伤性脑损伤后凝血功能障碍的发生机制和治疗进展

摘    要:创伤性脑损伤与凝血系统的功能变化有密切的联系,然而对两者间相互影响的机制尚缺乏深入研究。颅脑创伤后的高凝状态、低凝状态可导致微血栓形成或加重颅内出血,从而引起一系列的继发损伤。为此,研究者们提出了多种假说,如组织因子的释放、弥散性血管内凝血、纤溶亢进和血小板功能障碍等。该文针对颅脑创伤后凝血功能异常的发生机制,以及临床诊治和预后进行综述。

Pathogenesis and treatment progress of coagulation dysfunction after traumatic brain injury

Abstract:There is a close relationship between the traumatic brain injury and the functional changes of coagulation system. However, there is still a lack of in-depth study on the mechanism of the interaction between the two. The high coagulation state and low coagulation state after traumatic brain injury can lead to the formation of micro thrombosis or aggravate intracranial hemorrhage, which can lead to a series of secondary injury. Therefore, researchers have proposed several hypotheses, such as tissue factor release, disseminated intravascular coagulation, fibrinolysis and platelet dysfunction etc. This article reviews the pathogenesis, clinical diagnosis, treatment and prognosis of the abnormal coagulation function after traumatic brain injury.
