胃癌淋巴结微转移的检测及其意义

目的　探讨胃癌淋巴结微转移的临床病理意义。方法　对 60例胃癌患者的 2 187枚淋巴结应用抗上皮细胞膜 (EMA)单克隆抗体进行免疫组织化学染色。结果　被检病例的 61 7% (3 7/ 60 ) ,淋巴结的 3 6% (79/ 2 187)存在淋巴结内癌的微转移。微转移与肿瘤分化程度、浸润深度密切相关 (P <0 0 5)。微转移阳性组的 5年生存率(2 8 6% )明显低于阴性组 (79 8% ) (P <0 0 5)。结论　淋巴结微转移参与了胃癌的发展过程 ,并与胃癌的部分生物学行为有关 ,可作为预测肿瘤转移和评估患者预后的指标

The determination of lymph node micrometastases and clinicopathological signifcance in patients with gastric carcinoma

Objective To detect micrometases in lymph nodes and its relationship with clinicopathological features in gastric carcinoma Methods Paraffin blocks of 2 187 surgically removed lymph nodes and carcinoma tissuses from 60 cases of gastric carcinoma were re examined by immunohistochemical staining, using monoclonal anti EMA ResultsMicrometastases was observed in 37 of 60 patients (61 7%) and in 79 of 2187 lymph nodes(3 6%) examined Micrometastases was significantly correlated with the differentiated degree and depth of invasion (P<0 05) The 5 year survival rate was lower in positive groups of micrometastases (28 6%) than in negative groups(79 8%) (P<0 05) Conclusions Lymph node micrometastases may paly an importantrole in the development of gastric carcinoma and be related to partial biological behavior Itmay serve as marker for predicting the metastases and evaluating the prognosis of gastric carcinoma
