大剂量尿激酶治疗急性心肌梗塞疗效分析

为观察大剂量国产尿激酶（UKTP）静脉溶栓治疗急性心肌梗塞（AMI）的临床有效性及安全性。152名急性心肌梗塞患者接收大剂量UKTP治疗。其中118例给予200万U，34例给予300万U，对其中41例溶栓后90分钟进行冠状动脉造影。结果显示：梗塞相关血管再通率为76．3％；90分钟冠状动脉造影开通率为80．5％；住院死亡率为5．3％；轻度出血发生率为14．5％；无大出血及脑卒中发生。比较200万U和300万U的临床疗效，结果显示300万U血管开通率及轻度出血发生率均有增加的趋势。结论：大剂量UKTP治疗AMI安全、可能提高血管开通率。

Clinical Analysis of Thrombolytic Therapy Using High-dose Urokinase Tech-Pool in Acute Myocardial Infarction

The purpose of the study is to determine the clinical efficacy and safety of intravenous thrombolytic therapy using high-dose urokinase Teeh-Pool in acute myocardial infarction (AMI). A total of 152 patients ware treated with high-dose UKTP intravenously, 118 patients treated with 2 millionU and 34patients treated with 3 millionU,of which 41 patients underwent coronary artery angingraphy (CAG) at the 90th minute after begining of thrombolysis. The result show that the patency rate of IRA was 76. 3%by the clinical standards and 80. 5% by the 90th minute CAG.The total mortality was 5. 3% during hospitalization. The incidence of minor bleeding was 14. 5%; no patient suffered from major bleeding and intracranial hemorrhage. Comparing clinical efficacy of 2 millionU with 3 millionU,there were not singnificant difference in patency rate,mortality and bleeding complications. Conclusion:high-dose UKTP therapy could increase patency rate of IRA and was safe for AMI.
