自体组织移植修复气道缺损的实验研究

目的：探讨用自体支气管修复气管缺损的可行性。方法：取10只杂种犬切除其左主支气管，埋入自体大网膜中。2周后二次开腹开胸，用带网膜蒂的左主支气管段替代一段气管，观察移植物及吻合口愈合情况。结果：除2犬于Ⅱ期手术时死亡外，其余8只均存活至观察期满(3个月)，气管镜及组织学检查，见吻合口愈合良好，移植物血供丰富。结论：本动物实验表明，采用分期自体左主支气管移植、修复气管缺损是可行的。

EXPERIMENTAL STUDY ON REPAIR OF TRACHEAL DEFECTS BY TWO-STAGE AUTOTRANSPLANTATION

Objective:To evaluate tracheal reconstruction with revascularized left main bronchus that was implanted heterotopically into the greater omentum 14 days ago.Methods:In ten canines, the left main bronchi was harvested and implanted into the greater omentum. Fourteen days later, a 6 cm segment of the thoracic trachea was resected and the defect was repaired by transplantation of the revascularized autografts of left main bronchi with omental pedicle. The autografts and anastomosis were examined bronchoscopically for 3 months. Thereafter, light microscopic study on the grafts and anastomosis were done to evaluate the sufficiency of blood supply.Results:Two canines died of ileus 29, 40 days postoperatively respectively. Eight dogs survived until they were sacrificed 3 months postoperatively. Bronchoscopic and histological examinations demonstrated good healing of the anastomosis, intact epithelium and cartilage and sufficient blood supply.Conclusions: The animal study shows that it is feasible to repair tracheal defects with main bronchus by a 2-stage autotransplantation procedure.
