新生儿败血症病原菌及耐药性观察

摘    要:目的 了解我院新生儿败血症病原菌的分布及药物敏感性状况。方法 对1993年1月至2004年3月住院的83例新生儿败血症的临床资料进行回顾性分析。结果 83例共分离出病原菌84株,其中革兰阳性球菌62株;革兰阴性杆菌22株。革兰阳性球菌以凝固酶阴性葡萄球菌占优势,其次是金黄色葡萄球菌;而革兰阴性杆菌以大肠杆菌居多。葡萄球菌对青霉素几乎100%耐药,对红霉素、苯唑青霉素耐药率均超过70%,敏感药物主要有万古霉素、头孢哌酮/舒巴坦、头孢哌酮。革兰阴性杆菌对氨苄青霉素100%耐药,敏感药物主要为丁胺卡那和头孢哌酮/舒巴坦。结论 资料显示新生儿败血症的致病菌主要为凝固酶阴性葡萄球菌.对常用的抗菌药物有程度不同的耐药性。

Abstract:Objective To understand the distribution of pathogen of neonatal septicemia in local area and drug sensitiveness and help diagnosis and provide evidence for reasonable use of antibiotics. Methods Clinical data from 83 inpatients of neonatal septicemia during January 1993 to March 2004 were retrospectively analysed. Results 84 Pathogenic strains were isolated from 83 specimens. Among them, 62 strains were gram-positive cocci and 22 strains gram-negative bacilli. Among gram-positive cocci, the most dominant organism was coagulase negative staphylococcus, and the next was staphylococcus aureus; While among gram-negative bacilli, Escherichia coli were the most frequently isolated. Staphylococcus were up to 100% resistant to penicillin but more than 70% resistence rate to erythromycin and oxracillin. The resistance rate of gram-positive cocci to vancomycin, cefoperazone/sulbactam, cefoperazone were rather lower. The resistance rates of gram-negative bacilli to ampicillin were almost 100%. And toamikacin, cefoperazone/sulbactam were the lowest. Conclusion Coagulase negative staphylococcus is the primary pathogen for this district. Drug resistance varies in different bacteria.
