内固定治疗前臂双骨折疗效观察

目的：比较三种手术方法治疗前臂双骨折的并发症发生率及功能恢复结果。方法：对96例前壁新鲜双骨折进行回顾性研究。其中双针加植骨术32例，双针内固定36例，双板内固定28例。术后平均随访时间为1年。定期拍摄正侧位X光片，测量肘、腕关节及前臂活动范围，将临床及X城结果进行综合评价。结果：结果优良率如下，髓内针十植骨组88％，随内针组81％，钢板组85％。并发症分别为骨不连接、尺挠骨交叉愈合、骨髓炎、神经损伤、前臂旋转动能受限。结论：髓内针内固定加一期植骨术是治疗前臂新鲜双骨折的较好方法。一期植骨对促进骨折愈合、减少并发症发生具有重要意义。

A Rctrospective Study of Internal Fixation of Both-bone Forearm Fracture in Adult

Objective: To compare the rate of complications and functional outcome of three surgery methods for both-bone forearm fractures in adult. Methods: A retrospective review of 96 adults with both -bone forearm fractures treated withinternal fixation is presented. Intramedullaty fixation with bone grafting was performed in 32 cases, without bone grafting in 36cases and plate -and-screw fixatior was performed in 28 cases. During and average l- year follow- up period, range of motionof the elbow, wrist, and forearm was measured, and anteroposterior and lateral radiographs were obtained, then made the finalassessment. Results: Excellent results were obtained in 88% of patients in group of intramedullary fixation with bone grafting,81 % in group of intramedullary fixa ion without bone grafting, and 85% in group of plate-and-screw fixation. Complicationsincluded nonunion, osteomyelitis, svnostosis, neuropathy and rotational deformity. Conclusion: Intramedullary fixation withbone grafting provided a successful method for restoring optimum function and reducing the rate of complications after acute both- bone forearm fractures in adult.
