食管支架在食管良恶性狭窄中的应用

摘    要:目的探讨食管支架对食管良恶性狭窄的治疗效果及可能出现的并发症和处理方法。方法选取自2005年1月至2011年12月在江苏省肿瘤医院就诊的食管狭窄患者98例,根据病变情况选择支架,观察患者置入支架后吞咽困难的改善情况、并发症及其处理。结果 98例患者中,95例一次性成功置入支架,位置准确;2例位置偏低,使用支架调取器调整到正确位置;1例取出并重新置入;11例不能常规置入导丝,经积极处理后全部成功置入支架。10例支架置入术后再狭窄;3例出现食管瘘;5例支架下移。术后1周,患者吞咽困难症状显著改善(P<0.05)。结论内镜下置入食管支架操作简便,近期疗效好,并发症少,但应重视严重并发症的预防及处理。

Abstract:Objective To explore the efficacy of esophageal stent on benign and malignant esophageal stricture and its possible complications and the corresponding treatments.Methods Esophageal stents were put into 98 esophageal stenosis patients selected from Jiangsu Cancer Hospital from Jan.2005 to Dec.2011 according to their pathological changes,and the dysphagia symptoms improvement after implant and complications and treatment were observed.Results In 98 patients,95 cases were one time successful with accurate location.2 cases were lower positioned,which was adjusted to the correct position.1 case was removed and re-implanted.11 cases could not have routine placement of the guiding wire,but all were implanted successfully after positive treatment.10 cases of restenosis after implantation occurred.3 patients had esophageal fistula,stents in 5 cases moved down.Dysphagia were improved significantly one week after surgery(P < 0.05).Conclusion Endoscopic esophageal stent implantation is simple,with good short-term effect and fewer complications,but attention should be paid to the prevention and treatment of serious complications.
