ICU因外科手术后脓毒症死亡患者的临床特征分析

目的分析ICU外科手术后脓毒症死亡患者的临床特征。方法回顾性分析2007年7月至2012年7月首都医科大学附属北京友谊医院ICU诊治的91例因外科术后伴脓毒症死亡患者的临床资料。结果本组同期收治外科来源患者1 625例,其中术后因脓毒症死亡91例,病死率达5.6%。从受累及的系统、器官来分析,普外科:受累主要在肺脏、肾脏、消化、循环和凝血系统;胸外科:肺脏的问题明显突出;泌尿科:主要出现肾脏、肺脏、循环和凝血系统的问题;妇产科:出现肺脏、肾脏、循环和凝血系统的问题;血管科和骨科:出现心脏、肾脏、循环和凝血系统的例次相对较多。结论外科术后脓毒症患者的高病死率应该引起临床医师的重视,并采取积极措施进行救治。

The Clinical Characteristics Analysis of Deaths of Post-operative Sepsis in ICU

Objective To analyze the clinical characteristics of patients who died of post-operative sepsis in ICU.Methods Retrospective analysis of clinical data of 91 cases who died of post-operative sepsis in ICU of Beijing Friendship Hospital Affiliated to Capital Medical University from 2007 to 2012 was done.Results Ninety-one cases died from post-operative sepsis among 1 625 cases received from the surgery department during the same period,which made a mortality rate of 5.6%.Analysis from the affected organ and system,department of general surgery: the major affected were lung,kidney,digestive,circulation and coagulation system;department of thoracic surgery: lung problems was highlighted;department of urology: mainly the kidney,lung,circulation and coagulation system;department of obstetrics and gynecology: lung,kidney,circulation and coagulation system problems;vascular and orthopedics department: heart,kidney,circulation and blood coagulation system cases were more.Conclusion The high mortality induced by post-operative sepsis should be concerned by the clinicians to deliver active treatment.
