CT引导下胸部大血管旁小病灶穿刺活检术的应用

目的：评价CT引导经皮穿刺胸部大血管旁小病灶的临床价值。方法：CT引导经皮穿刺胸部大血管旁小病灶127例，分析其敏感性、特异性、准确性及并发症。结果：恶性肿瘤的诊断敏感性为85．9％（79／92），阳性预测值为100％，良性病变诊断特异性为100％，阴性预测值为72．9％（35／48），总诊断准确率为89．8％（114／127）。气胸发生率为7．9％（10／127），肺内出血发生率为10．2％（13／127），均未作特殊处理自愈。病灶周围有肺气肿是气胸及肺内出血的危险因素（P〈O．05），而病灶大小及深度均与上述并发症无关（P〉0．05）。结论：CT引导经皮穿刺胸部大血管旁小病灶准确性较高而并发症较低。

APPLICATION OF CT - GUIDED PERCUTANEOUS BIOPSY ON CHEST SMALL NODULES NEAR BIG BLOOD VESSEL

Objective To evaluate the clinical value of CT-guided percutaneous biopsy on chest small nodules near big blood vessel.Methods 127 patients underwent CT-guided percutaneous biopsy on chest small nodules near big blood vessel.Sensitivity,specificity and accuracy were calculated,and complications were analysed.Results The diagnostic sensitivity and positive prediction value of malignant diseases were 85.9%(79/92)and 100%.The diagnostic specificity and negative prediction value of benign diseases were 100% and 72.9%(35/48).The diagnosis accuracy was 89.8%(114/127).The occurrence of pneumothorax and bleeding were7.9%(10/127)and 10.2%(13/127).Emphysema near the lesion was a significant factor related to the occurrence of complications in the process of CT-guided percutaneous biopsy on chest nodules(P<0.05).There was no significant difference among the complication rates with the size and depth of the lesion(P>0.05).Conclusion CT-guided percutaneous biopsy on chest small nodules near big blood vessel has high diagnostic accuracy and low complication incidence.
