摘    要:目的 提高CT诊断卵巢转移肿瘤的准确性。方法 对33例经病理证实的卵巢转移癌的CT检查及临床资料作回顾性分析。结果 CT表现分为3种类型:囊肿型、肿块型和混合型,分别占总数的12.1%(4/33)、24.2%(8/33)和63.6%(21/33)。消化道肿瘤转移占78.8%(26/33)。双侧卵巢转移瘤占42.4%(14/33),原发灶在乳 及上腹部脏器者占71.4%(10/14),在下腹部脏器者占28.6%(4/14);单侧卵巢转移瘤占57.6%(19/33),原发灶在上腹部脏器者占10.5%(2/19),在下腹部脏器者占84.2%(16/19)。结论 结合临床病史及体征,CT有助于提高诊断卵巢转移肿瘤的准确性。

Abstract:Objective To improve the diagnostic accuracy of metastatic ovarian cancer by CT scan. Methods CT scans of 33 patients with pathiologically proven metastatic ovarian cancer were retrospectively reviewed. Results Metastasis form cancer of gastrointestinal tract was most common (78.8% 26/33). Bilateral ovarian metastasis were seen in 14/33 cases ( 42.4 % ), among which 5 were from colon, 5 from stomach, 2 from liver and 1 breast, 1 from pancreas, 19/33 were unilateral ovarian metastases(57.6% ), among which, 9 were from the rectum, 6 from colon, 2 from stomach; 1 from uterus and one of unkown origin. The patterns of lesions on CT scans were cystic, mass like and mixed, which were 12.1% (4/33), 24.2 ( 8/33 ) % and 63.6 % (21/33), respectively. Among the bilateral ovarian metastases, the primary sites tended to be originated from the upper abdominal organs (71.4 % ), whereas in the unilateral lesions, the primary site tended to be originated from the lower abdominal organs(84.2 % ). Conclusion CT is useful in diagnosing ovarian metastases cancer precisely. (Shanghai Med J, 2001,24:140)
