尼莫地平治疗动脉瘤性蛛网膜下腔出血疗效观察

本文作者临床观察了用尼莫地平治疗动脉瘤性上腔出血的３３例病人。尼莫地平的应用方法是通过微量泵静脉注入，最初剂量为０．５ｍｇ／ｈ，每６ｈ增加量，直到达到２ｍｇ／ｈ，维持３周，持续监测动脉压和心电图，入院３个月后统计病人的预后情况。良好５７％，中残１５％，重残１２％，死亡１６％，比过去的文献统计动脉瘤性蛛网膜下腔出血的预后明显改善。

Effects of Nimodipine in the Treatment or 33 Patients with Aneurysmal Subarachnoid Hemorrhage

33 Patients of aneurysmal subarachnoid haemorrhage were managed with same Ni-modipine treatment protocol. After confirmation of SAH the Nimodipine treatment was started ascontinuous intravenous perfusion at a dosage of 0. 5 mg/h and gradually increased every 6 hoursuntil the maintenance dose of 2 mg/h was reached. Median systemic pressure was continuouglymeagured and ECG was continuously monitored the full time of Nimodipine application was 3weeks. Then we evaluate the prognosis of Nimodipine treated patients in 3 months after admis-sion. We found good recovery 57 %, moderate disability 15%, severe disability 12%, death 18%.The results of our group of SAH with Nimodipine-treated was very well than that of historicalcontrols which without Nimodipine-treated. We therefore recommend Nimodipine administrationto these patients of aneuryamal subarachnoid haemorrhage.
