简易器械选择性输卵管造影及再通术诊疗不孕症的临床应用研究

目的：探讨自制简易器械选择性输卵管造影及输卵管再通术的临床应用价值。方法：经常规输卵管造影诊断为输卵管阻塞者86例，除2例外均成功进行了选择性输卵管造影，63例确诊输卵管阻塞者施行了再通术。结果：本方法插管成功率97．7％，造影证实20例双侧输卵管完全通畅，消除了假阳性。63例中选择性输卵管造影发现98条输卵管不通，其中8．4条疏通，再通总成功率85．7％。其中间质部与峡部阻塞复通成功率92．5％，壶腹部阻塞复通成功率83．3％，漏斗部及伞部复通成功率28．6％。术后随访受孕率30．1％，未发现严重并发症。结论：本方法诊疗输卵管阻塞及时、准确、简便，再通率高，是输卵管阻塞性不孕症患者首选的一种安全、有效、快捷的诊疗方法。

Clinical study of simplify apparatus selective salpingography and oviduct recanalization for diagnosis and treatment of the unconception

Objective:To explore the diagnosis value of selective salpingography and the clinical effect and value of oviduct recanalization with home-made simplify catheter.Methods:86 cases of oviduct obstruction was diagnosed by routine salpingography,the selected salpingography were successfully performed in all patient except 2 cases.63 cases of patients with obstruction of the oviduct were recanalized.Results:20 cases of patients with normal oviduct were discovered by the method above mentioned and the false positive was also expulsion. In 63 patients,98 oviducts were confirmed jam through selective salpingography.Of which 84 were successfully recanalized,the rate of successful recanalized was 85.7%,the rate of successful catheterization was 97.7%,in uterine part and isthmus part ,the rate of recanalized was 92.5%. In ampulla part,the rate of successful was 83.3% and infundibular part and fimbria part was 28.6%. All cases were followed up ,the rate of pregnant was 30.1%,and no serious complication was found.Conclusion:This method for diagnosis and treatment of obstruction of the oviduct is safe,simple and accurate,the successful rate of recanalized is high. It's the first choice to the infertility patients.
