乌梅丸治疗心血管神经症伴顽固性失眠临证体会

目的]探讨运用乌梅丸治疗心血管神经症伴顽固性失眠的临证经验。方法]通过临床跟诊及病房诊治,收集相关资料,从心血管神经症伴顽固性失眠的中西医认识,及其病机与乌梅丸方证病机对应等方面来论述相关学术经验及临证心得,并分析典型案例。结果]临床上心血管神经症伴顽固性失眠患者辨证多为肝阳虚夹郁火的寒热错杂证,而乌梅丸作为厥阴病主方,功擅温肝寒、益肝体、泄郁火,高度契合病机。所举验案病机为七情戕伐阳气导致肝寒气馁、肝气郁滞而化火,治以乌梅丸为主方,随证加减,寒热同调,补泻兼施,收获良效。结论]运用乌梅丸治疗心血管神经症伴顽固性失眠的疗效显著,对于中医临床工作有一定指导意义。

Experience of Application of Wumei Pill in the Treatment of Cardiovascular Neurosis Accompanied with Intractable Insomnia

Objective]To discuss the academic ideas and clinical experience of using Wumei Pill in the treatment of cardiovascular neurosis accompanied with intractable insomnia.Methods]Through collecting the relevant information through clinical study and diagnosis and treatment of inpatients, the academic ideas and clinical experience of using Wumei Pill was discussed in a manner of comparing the pathogenesis of cardiovascular neurosis accompanied with intractable insomnia and Wumei Pill, which based on cognition of traditional Chinese and western medicine. And a typical case was presented.Results]Clinically, most patients who have cardiovascular neurosis accompanied with intractable insomnia belong to cold-heat complicated syndrome due to the deficiency of liver Yang with stagnated fire. As the main treatment plan of Jueyin disease, Wumei Pill specializes in warming the liver, nourishing the liver and leaking out the stagnated fire from liver, and it is highly compatible with the pathogenesis of this disease. It was found that excessive emotions lead to liver Yang weakness and liver Qi stagnation and transforming into fire, which was the main pathogenesis. After treated with Wumei Pill as primary method, modified according to symptoms, balanced cold and heat syndrome, tonified and purged in combination, the patient achieved good results.Conclusion]The clinical experience of using Wumei Pill in the treatment of cardiovascular neurosis accompanied with intractable insomnia is significant, and it maybe performs well in the clinical work of traditional Chinese medicine.
