16例老年脑梗死患者的药学监护

目的:探讨老年脑梗死患者的药学监护特点,协助医师,确保患者用药安全。方法:16例老年脑梗死患者入院后给予清除氧自由基、抗血小板聚集、营养神经等药物治疗。临床药师实施药学监护,关注药物的合理使用,参与药物治疗方案的调整,提出合理化建议,同时密切观察患者药品不良反应,并对患者进行出院指导。结果:本组16例患者平均住院天数18d,发生不良反应8例,经治疗均好转出院,无死亡。结论:临床药师加强老年脑梗死患者的药学监护,可促进合理用药。

Pharmaceutical Care for 16 Elderly Patients with Cerebral Infarction

OBJECTIVE：To investigate the characteristics of pharmaceutical care for elderly patients with cerebral infarction and assist physician to guarantee the safety of drug use.METHODS：16 elderly patients with cerebral infarction after admission received treatment of scavenging oxygen free radicals,anti-platelet aggregation,nutrition for nerve and other drug treatment.Clinical pharmacists provided pharmaceutical care,paid attention to rational use of drugs,participated in adjustment of drug treatment programs,and made reasonable suggestions,while observed adverse drug reactions closely,and provided guidance for patient discharge.RESULTS：For 16 patients in the group,there was an average length of stay of 18 d,8 cases of adverse drug reactions.They discharged after treatment,and no death was found.CONCLUSION：Clinical pharmacist should strengthen the pharmaceutical care for elderly patients with cerebral infarction,and promote rational drug use.
