免疫金银染色法用于检测系统红斑狼疮患者血清抗核抗体和抗双链DNA抗体

目的 探讨检测抗核抗体（ANA）、抗双链DNA抗体（A－dsDNA)对系统性红斑狼疮（SLE）的诊断意义。方法 采用免疫金银染色法（IGSS）和用斑点免疫金银染色法（Dot-IGSS)，检测SLE病人血清中ANA和A－dsDNA。结果 113例活动期SLE患者血清ANA阳性率97．3％，A－dsDNA阳性率71．7％；35例非活动期SLE患者血清ANA阳性率74．3％，A－dsDNA阳性率14．3％。活动期ANA、A－dsDNA阳性率与非活动期比较均有显著性差异（P＜0．01）。活动期ANA平均滴度明显高于非活动期，A－dsDNA阳性组肾损害明显高于A－dsDNA阴性者（P＜0．01）。结论 IGSS和Dot-IGSS检测ANA、A－dsDNA有高度特异性、敏感性与稳定性，较高滴度A－dsDNA提示有肾损的可能。

Detection of ANA and A- dsDNA antibodies in the sera of patients with SLE by IGSS and Dot- IGSS

Objective To detect the anti-nuclear antibody (ANA) and anti-double stranded DNA(A-dsDNA) in the sera of patients with SLE. Methods The ANA and A-dsDNA antibodies in the sera of 148 patients with SLE were detected by using immunogold-silver staining (IGSS) and Dot-IGSS. Results The positive rates of ANA and A-dsDNA were 97.3% and 71.7% respectively in 113 active cases of SLE, and 74.3% and 14.3% respectively in 45 inactive cases. The difference in the positive rates of ANA and A-dsDNA was significant between the active and inactive cases (P<0.01), and the titers of ANA in active cases were higher than that in the inactive cases (P<0.01). The incidence of renal damage in patients with positive A-dsDNA was higher than that in patients with negative A-dsDNA. Conclusion The IGSS and Dot-IGSS are highly sensitive and reliable for detecting ANA and A-dsDNA in patients with SLE. And the higher titer of A-dsDNA suggest the possibility of renal damage.
